COVID-19 Status Update - 4/1/20 11:30AM
The Scott County Government Center building is closed to the public at this time, until further notice. However, we remain committed to serving our citizens to every extent possible.
The Home and Community Care Assessors and Case Managers will be conducting most business via phone and email. If you have questions, please reach out to your Assessor or Case Manager directly via phone and email. If you are not sure who your Assessor or Case Manager is, please call 952-445-7751 for assistance.
The Home and Community Care Programs help people live in the community as independently as possible. Our services can help the elderly and people with disabilities, and their families, with service planning and getting the help they need.
Paying for Services
During an assessment visit, our assessors will discuss insurance or other payment sources which could help with the cost of services. Some people are eligible for assistance with part or all of the cost of services through state and federal-funded programs administered through Scott County, such as Alternative Care (AC), Elderly Waiver (EW), Community Access for Disability Inclusion Waiver (CADI), Community Alternative Care (CAC) and Brain Injury Waiver (BI). Private pay options can also be arranged for eligible services.
Get an Assessment
The MnCHOICES assessment has been developed to help individuals make decisions about the supports and services that will help them stay in the community while focusing on their needs and preferences. To schedule an assessment, call our Home and Community Care/Disabilities Intake line at 952-496-8556.
