Ages 15 and Under

If you are 15 of younger you will need the following in order to obtain a passport:
  • Evidence of U.S. Citizenship
  • Evidence of Parental Relationship
  • Photo Identification
  • Parental Consent
  • Passport Photo
  • Application Forms (See Below)
  • Passport Fees
  1. Evidence of Parental Relationship

    You must submit evidence that lists you as the parent of the minor applying for a passport.

  2. Evidence of U.S. Citizenship

    You must submit one of several original certified documents as primary evidence of U.S. citizenship for your child to obtain a passport.

  4. Parental Consent

    Minors under age 16 cannot apply for a passport by themselves, they must have parental consent.

  5. Photo Identification

    When you submit your application, the parent (or child, if he or she has an ID in his or her own name) must present one of the following primary photo identification documents.
